dental requirements. We are pleased to welcome
you as a new patient and look forward to being

In order to
ensure that your first visit is as comfortable
and pleasant as possible, we
will require some information regarding your
dental and general health.
Upon arrival
at our office, you will be asked to fill out
a Patient
Information Form detailing your medical
/ dental history, and insurance coverage information,
as well as a Hygiene
Questionnaire to help us better understand
your dental needs.

of the forms that you can print at home, fill
out at your convenience, and bring with you
